Adding cyclic prefix in ofdm - QAM16

Hello,

I wanted to know, adding cyclic prefix is to each symbol or to an entire transmitted vector?

I found some references that say to add cyclic pref. to each symbol and others say to add to the transmitted vector.

Which one is more accurate?
