Issue regarding Error in port widths or dimensions

"Error in port widths or dimensions. 'Output Port 1' of Transition' is a one dimensional vector with 1 elements."
Team please help me I am facing this issue.

Such error message is almost always paired with a similar message about an input port, and that other message is important in understanding this one.
