Setting up a n-D lookup table in simulink

using simulink 7.5 (matlab 2010a), need to know how to set up a 4-D lookup table in the function block table window.

What exactly are you having problems with? How far does http://www.mathworks.com/help/simulink/slref/ndlookuptable.html help?
