How to construt the operation block as in the example

hello everyone
I just see the example provided by Mathwork named ""Wavelet Reconstruction and Noise Reduction"" and i want to build a block have the same function as that of the operation block in the example. how can i build such block
the model name is
dspwavelet.mdl
